Richard Reynolds represented Mr Tyler’s sister and was instructed by Jocelyn Cockburn and Claire Brigham of Hodge, Jones and Allen Solicitors.
Mr Tyler, who had complained to healthcare staff from 19 October 2016 of chest pain and difficulty breathing, and who had a systolic heart murmur, died of infective endocarditis on 31 October 2016. The jury identified a host of serious failings, including: poor keeping and consultation of medical records; inadequate clinical assessments between 19 and 24 October; failure to realise that drug withdrawal was no longer a factor; failure to give significance to the complaint of chest pain; and repeated failures to make a referral to hospital.
